1. Your first initial and surname
2. The incorporation date of your limited company
3. The date you ceased trading through EasyLimited
4. The date(s) you pay your monthly accounting fee
5. Your current EasyLimited membership level and status
6. Whether or not your monthly accounting fee payment was successful or not
